To Apply for National Certification Testing
At this time you can only apply through the mail. The following forms cannot be
transmitted over the internet, you must print and mail these documents.

Print the application. (You can fill in the blanks
online and then print from your browser or print a blank form and fill in by hand. Note:
If you fill in the form online and print, verify that everything you enter in the
scrolling text boxes appear on the printed form.)
Gather together a copy of your school certificates. Print the "Authorization to Release Educational Records"
AND "School
Verification Form" forms. Fill it out and send it to your school or instructor with a note asking them
to send your transcript directly to ARCB. If no transcript is available the school/
instructor may send a letter verifying the number of hours of instruction with the date
you completed the program. The letter, on stationary, must include a breakdown of the
subjects in the curriculum; the number of hours devoted to each subject, and a person to
contact if more information is needed. Send the application with your certificates and check or credit card
authorization for the testing fee of $295 to:

Upon receipt of the application funds and your school records the study guide outline
will be sent to you along with other forms that are part of the certification examination.
